If possible please return the check that is in the name of the decedent along with the completed Beneficiary Declaration and Release form. The failure to return the original check will not affect your request but it will slow down the refund process because the Settlement Administrator will need to confirm that the check wasn't cashed and will have to cancel the prior check.
